Center for Connected Health joins our Fall 2012 Accountable Care and Health IT Strategies Summit in Chicago

We are pleased to announce that Robert Havasy will be joining our Fall 2012 Accountable Care and Health IT Strategies Summit in Chicago, Nov 27-29.
As the Technical Architect and Operations Manager for the Center for Connected Health (CCH), Robert Havasy will discuss integrating the Center’s Remote Monitoring Data Repository with Partners Healthcare’s core clinical systems to provide broad data accessibility:
By making remote monitoring data available to all clinicians through Partners’ ambulatory medical record, and to patients through Partners’ Patient Gateway portal, CCH has elevated remote monitoring programs from pilot to full scale. CCH’s remote monitoring programs are a key piece of Partners’ Accountable Care strategy, allowing Partners to monitor whole patient populations between traditional episodes of care.

By leverging information technology – cell phones, computers, networked devices and simple remote monitoring tools – our connected health programs are helping providers and patients manage chronic conditions, maintain health and wellness, and improve adherence, engagement and clinical outcomes.
