State Innovation Model Transformation: Analytics, Risk and Performance Management Capabilities
Panelists present an overview of the SIM program progress nationally and the alignment of HIT, HIE and data analytics in supporting overall health delivery system transformation. Colorado present the state perspective on coordination of HIT transformation across providers in the state, the centralized data aquisition, aggregation, analytics and reporting strategy as well as plans to expand broadband connectivity and Telehealth. ECG present the work undertaken with Illinois Partnership for Health, Illinois largest ACE (Accountable Care Entities in terms of HIT and data analytics preparedness to support their participation in coordinated care in a rapidly evolving payer and risk environment.
Q&A will focuses on the successes, challenges and HIT road map moving forward to ensure providers and payers a like with the tools they’ll need to be successful under rapidly evolving risk and contracting landscape.
