The next generation of population health management—a linchpin of healthcare reform—will require much more than remote measures of health trends. Now, integrated programs are needed that will improve community health and provide measurable statistical results and reduce healthcare costs. Success will depend on a shift in focus from the clinic to the population at large, and adapting provider practices, data collection and analysis.
