Track 1 | Partners Healthcare Case Study: Bridging the Gap: Integrating Remote Monitoring Data Into Core Clinical Workflows

Robert Havasy, the Technical Architect and Operations Manager for the Center for Connected Health (CCH) will discuss integrating the Center’s Remote Monitoring Data Repository with Partners Healthcare’s core clinical systems to provide broad data accessibility. By making remote monitoring data available to all clinicians through Partners’ ambulatory medical record, and to patients through Partners’ Patient Gateway portal, CCH has elevated remote monitoring programs from pilot to full scale. CCH’s remote monitoring programs are a key piece of Partners’ Accountable Care strategy, allowing Partners to monitor whole patient populations between traditional episodes of care.

Track 2 | Connected Communities: Alere Wellogic’s successful HIE Expansion with Hawai’i Island Beacon Community

Alere Wellogic’s President & CEO Sumit Nagpal will talk about Wellogic’s involvement  in the Hawaii Island Beacon Community Project,  bringing a community of unrelated practice hospitals that compete into a cohesive exchange in a short amount of time.  This informative session will provide insight on how to align incentives across competing organizations, cover stepping stones to HIE implementation and key factors for adoption.
